Reviews of a RAP should be completed every 12 months, or sooner when any change in condition, move of location, or change of role occurs.
The RAP document can look daunting, but it asks for details of any condition, any medication or treatment and how these effect the person. It is then for the individual to state what they need.
A RAP is up to the individual to complete as some may decide they do not want or need one.
When putting this together a discussion with line manager is often helpful as what is reasonable and possible given each role and situation can be discussed. It is for the individual to decide what they feel is relevant and what they wish to disclose. It is worth noting that the organisation cannot be responsible for what they do not know about.
