Police Treatment Centre - New application Process

As of February 2024, The Police Treatment Centres will be accepting Patient Summaries to accompany applications for residential physiotherapy at both of our Centres.
Anyone submitting an application form for Residential Physiotherapy Treatment will no longer need to get the form signed by their GP on pages 4 & 5 of the application form
Instead you now need only request your 'GP health records' from your local surgery which is a simple administrative task

This page also contains a letter that patients can use to request their patient summary from their GP Surgery.
