GROUP INSURANCE TRAVEL CLAIM FORMS

Due to the understandably high volume of calls and claims to the Travel Claims team - Philip Williams have provided claim forms for making your claims directly.

A selection of travel claim forms is available for download from the links shown on the Philip Williams website.

Please complete the forms and send them to the address shown at the top of each document.

There is also a template letter that may assist in the event of difficulties obtaining refunds from tour operators and travel agents. Two versions are available - MS Word and PDF should your browser not allow you to download a Word document.
