If you’d like to apply to join the Group Insurance scheme, or would like to update or amend your beneficiary details, please complete the below forms and return to the Federation Office

If you are unable to complete the Health Declaration on the Application Form, please complete Underwritten Application Form

Amend Or Change Your Beneficiaries

Should members or their partners require additional life or critical illness insurance, please complete the supplementary health declaration application form and return it directly to Philip Williams & Co as stated on the application form.

If you are unable to sign the health declaration, please complete the supplementary underwritten application form instead.
